Year-end medication pick up notice

Dear Parents/Guardians:
 
If your child has any medication in the office of the health and wellness coordinator, it must be picked up by Friday, June 1, 2018.
 
Any medication/spacers not picked up from school will be discarded.
 
At The Varnett Public School, medication is always transported from home to school and back by adults. This is to prevent any misusage of medication by children.

It is important to note that a new medication/allergy form must be completed and signed by a physician for the upcoming 2018-19  school year and returned only to the campus health and wellness coordinator.  All prescription medication must be submitted in the original prescription bottle/package and clearly labeled.
 
No over-the-counter medication is ever administered by the health and wellness coordinator. All medical questions and concerns must be communicated to the health and wellness coordinator at all times.
